is there anyone know how to make this kind of report.

what i need is:
the report will ask for:
1.month
2.year
3.week(week 1 to 6)
note: dec 1, 2012 is not a part of the first week of dec. it is group with the 4th week of nov 2012
-the 5th week of dec2012 includes (30,31, jan 1, jan 2, jan 3 to 5)

you can look into your calendar to understand.

or this kind of report:

the report will ask for:
1.year
2. week 1 to 5 with corresponding date range
ex:
week 1 (jan 1 to jan 7)
week 2 (jan 8 to jan 14)
3. better if include month
