I'd like to find out how to create a report that shows customers who paid their membership after the promo ended.
 
For example:  A fitness center
 
A new customer is offered 2 free months of membership.  The renewal amount after the 2 free months if $99 dollars.
 
I need to create a report that shows how many customers paid (continued the membership) the subsequent fee of $99 after the first 2 free months. 
 
I'd like the report to show:
 
Customer Name   January 2008 February 2008 March 2008  April etc.... 
John Adams            0                       0                   99                 99
Steve Smith            0                       0                   99                  0
total                        2                       2                       2                1
 
I'd appreciate any help from anybody,

How is the date field stored in database? Do you have a field for every month or it is just one field?

Can you post some sample data so that I know how it appears in the database? If you know how to create a cross tab report with SQL, then you might be able to do this. But there might be other ways to do this.
