I can run a report with Crystal Version 11..0.0.1282 to search for patients with a specific diagnosis, within a specific date range, but the report returns patient names, service date, and patient may show twice if seen more than once in same year. How do I change query, or under select patient record, so that report will return only unique visits during time interval, not duplicates of same patients seen more than once? I'm somewhat familiar with Boolean commands and SQL fields from the Electronic Health Record, where I pull the data.  Any help greatly appreciated. I am new to this forum.

you can filter by Patient MRNo (Medical Record Number) and Patient Episode (admission number when the Px check-up in hospital).
 
e.g.:
MRN = 12345
Episode =
- EM003823  (emergency episode)
- OP002828  (outpatient episode)
- IP012387   (inpatient episode)
 
in this case it will give you the exactly episode for the patient that you search.
and no need date-range.
 
if you need date range, it will give you double episodes for 1 patient (if he/she visit more than 1 time to the hospital).
in this case you will need the Patient episode as well to be printed in the report (to identify the date and so-on).

MY EHR is Chartlogic, an office based, not hospital based program, so the suggestions don't exactly apply. Each patient does have a unique ID field, so it does make sense to filter for unique ID of patients seen within a specific date range. I'm a beginner at Crystal language and reports, so am uncertain which part of Crystal Reports you would use, such as Patient Record Selection, to filter on ID field, and what syntax to actually use.
 
All suggestions greatly appreciated.

oops.. i thought you using hospital system.
 
in this case you need to know the information that saved in your system (e.g.: PxMRN, DateDiagnose, Results, etc.).
after you find out those information than you can work inside CR.
 
in order to get a result that you want you need to put create some parameters (as you request, date range,etc.).
 
to create a params:
1. click 'View' menu
2. click 'Field Explorer'
    you will find some Main information for the fields.
    one of them is 'Parameter Fields'.
3. right click on 'Parameter Fields'
4. select 'New'
5. type parameter name (DateFrom, DateTo --> create it separately)
    where the Type is 'Date'.
6. click 'Ok'
 
 
steps to create selection record:
1. click on menu Report - Selection Formula - Record
    it will appear a window for 'Formula Workshop'.
 
2. in here you can type the filter as below.
    {tbl.servicedate}>={?DateFrom} and {tbl.servicedate}<={?DateTo}
 
3. close & save
 
and run the report by pressing F5.
the report will prompt you for the date.
 
till here you know how to create params in CR and filter your report based on the params.
 
Other option, if you know the SQL command to run the report then you can type those SQL command in CR.
e.g.:
select PxMRNo, DateSrvce, PxDiagnose, PxNote from PxResultTable
where DateSrvce>=('2010-04-01') and DateSrvce<=('2010/04/30')
